‘Bear with Intestines’ is a reference to the Bear bile industry where bears are kept alive in cages for the bile that they produce. The bile is drained twice daily from the caged bear’s gall bladder with a steel catheter. The wound is often infected and is never able to heal.
The image is painted entirely of 1 mm high words which outline the cruel practice.

Bear farms across China Korea and Vietnam hold thousands of wild and farm bred bears as part of the illegal bear bile industry
Bears are locked in cages barely able to move
They have a steel catheter inserted into their wound for twice daily bile extractions
The wound never heals is often infected and will cause the bear constant pain
The pain can be so intense some bears chew on their own limbs and suffer severe mental trauma
Some bears live in this existence for more than ten years
If a bear stops producing bile after a few years they are left to die or killed for their gall bladder and paws
The bile drained from these bears supplies an estimated $100 million annual global black market trade in bile products and bear gall bladders
The illegal bear bile farmed in Asia is illegally exported in medicine shampoo and wine
Cheaper and more effective herbal and synthetic alternatives to bear bile are available containing the active ingredient UrsoDeoxyCholic acid found in bear bile
